Dang, we are sad to see that one of our players has suffered an injury. Please fill out the form below to the best of your knowledge and click "submit" when finished. The form will be sent directly to CCYF staff for processing. If you are not sure how to answer one of the questions, do your best and then add a note in the "Describe Injury and Care Given" section.
